Frank Nordberg, the webmaster of Musicaviva, transcribed this in Finale, and gave me permission to put up his PDF of the score, with both the lute tab and a realization of it, with the cantus part. He puts more work into his MIDI files than I do -- here's the one for this transcription. I usually make my group play this before tackling the 5-part version.

Note that the ABC w: construct is insufficiently flexible to deal with this structure, where there's a chorus which appears before the four verses. So the ABC notes are the same as the lilypond ones, but I fixed up the underlay after the translation to lilypond.
For a more modern take, Erik Koerner transcribed this one in lilypond. Here's the lilypond, and here's the pdf for the score. For some reason, the parts Eric sent me won't convert to pdf on my machine, so here's a zip file with the parts in postscript. And here's the MIDI
